DescriptionElectroencephalogram (EEG) records include a request for an EEG examination detailing patient name, address, register number, ward or clinic, whether taking medication or receiving treatment, a clinical survey which gives details of their condition and reason for request, provisional diagnosis and details of the problem for investigation. If the patient has had previous examinations this is also recorded. Requests for an examination for out-patients are, in several cases, accompanied by a letter from the patient's G.P. On the reverse of the request form are the results of the examination.

Each file is arranged alphabetically by patient surname. It appears a dual filing practice has been adopted with regard to patients who have had more than one examination, with records, in some cases, being grouped together and arranged by date of first examination and in other cases by date of most recent examination.
